This white paper looks at the information on mishaps to find the causal factors involved with nationwide DoD Aircraft, Navy and Marine Maritime, and Train mishaps. The problems can be grouped into three main categories: human error, material failure/malfunction, and environment factors.
The majority of the Department of Defense’s (DoD) supply chain management is overseen by the Defense Logistics Agency (DLA).
